Applications and Legal Questionnaire

To apply to be a volunteer in CM, complete either the “Volunteer” or “Student Volunteer” form (for High School and Middle School students only) and “Policies and Procedures Agreement and Legal Questionnaire” and “Background Check” and return to Connie Lutter in the office. Membership at Trinity is not a requirement to volunteer. You must have attended Trinity for at least 6 months, attend an orientation class and complete the paperwork below.
